The Adult Social Day Program is Folak's licensed day program in Franklin Park — day habilitation and person-centered support for adults 21 and older with intellectual and developmental disabilities across six counties in New Jersey.
The day program provides day habilitation, community-based support, individual support, respite, behavior management, community inclusion, prevocational training, and transportation, each matched to a participant's Individualized Service Plan.
New Jersey DDD funds these services through Medicaid waivers — the Supports Program and the Community Care Program — and the team coordinates with each adult's Support Coordinator on eligibility and enrollment.
